Admission Procedures

  1. On the day your admission to the hospital, please come to the Admission Center by the indicated time. Please make sure that you or your agent completes the procedures for admission to the hospital before going to the Ward. Please prepare the following documents for admission to the hospital.

Documents necessary for all patients

  1. Patient registration card
  2. Health insurance certificate
  3. Seal
  4. Admission Application and Payment Guarantee Form
  5. Application to start and end leasing of patient's gown
  6. Bedding rental and return form
  7. Confirmation of hospitalization period

Documents necessary for patients who correspond to the following

  1. Claimant certification for all medical care
  2. Medical care ticket underthe Public Assistance Act or a written statement on the necessity of medicalcare
  3. A certificate for the reduction of meal and medical expenses during hospitalization
  4. A certification of discharge from another medical institute
  5. Co-pay partially available
  6. Exemption certificate (maximum applicable amount affidavit, etc.)
  7. Nursing insurance card
  8. Nursing services plan (care plan)
  1. Please note that if a health insurance certificate is not presented, you will be responsible for the cost of your medical treatment.
  2. Your health insurance certificate and claimant certification for each type of medical care will be returned immediately after being checked.
  3. When your qualification as an insured person changes or when any change is made to your claimant certificate for each type of medical care after you are admitted to the hospital, please notify the Admission Center promptly. In cases where medical care cannot be covered by the certificate since your notification is delayed, you will be responsible for the cost of your medical treatment during that period. If a new insurance certificate, etc. is issued, please present it to us on its issuance.Please present your insurance certificate, etc. at the beginning of each month of your hospitalization.
  4. Please note that you may be asked to change scheduled inpatients' ward.We appreciate your cooperation.

Items to be prepared

  1. The items to be prepared during your hospitalization will be explained in advance by a doctor or nurse. In addition, it may be helpful if you bring daily necessities with you; they are also available at the Kiosk in the hospital.

Clothes

  4. Earphone (You can buy it in a convenience store.)
  5. Artificial teeth storage case (If necessary)
* Please bring quiet, low-heeled, non-slip footwear that you have often worn before.
* Please note that slippers come off easily and are slippery, so you have a greater risk of falling.
  1. Please bring a clean patient gown. Patient gowns are also available for lease in this hospital; please request one if you so desire.
    Rent is 73 yen per day.
    The patient gown will be changed three times a week from July to September and twice a week from October to June.
  2. If you have your medication records, please bring them with you at your admission to the hospital. We will enter the records as needed.

Special Inpatient Rooms

  1. If you prefer to use a special inpatient room, please consult your medical care department on your admission to the hospital.
  2. Health insurance will not cover the room charges for special inpatient rooms and you will be required to pay full amount of the room charges.
  3. The following room charges are not calculated per night. For example, if you stay one night and two days in the hospital, the room charges will be for two days.

Issuing of certificates

In cases where you need certification of medical care, such as a medical certificate, certification of hospitalization, etc., or other kinds of certificates, please request them from the Certification Reception at the Outpatient Counter on the first floor.
* Medical certificates are issued by each ward/clinic, and fees apply.
